چکیده

Thank you for your constructive comments. We have complied with all revisions suggested by the editors and reviewers except in the case of the use of hyphenation for the term "African-American". As indicated in an earlier e-mail to the editor, there is disagreement regarding the appropriate use of this hyphenation. We have elected to comply with the generally accepted standard to use the hyphenated form only when the term is used as an adjective. There are minor revisions to the text for clarity. The final sentence of the abstract has been revised. Also, we have added an acknowledgement section at the request of one author to acknowledge grant funding. It was not clear where it should appear so it was added after the Authors Contribution sections. We have added an affiliation for one author. This is the extent of the revisions.
